Overview
Traveler's diarrhea affects 30–70% of international travelers (CDC). Ciprofloxacin is effective against the most common bacterial causes including enterotoxigenic E. coli, Salmonella, and Shigella. It remains the CDC's primary recommendation for most destinations outside South/Southeast Asia.
It is typically used as a single 750 mg dose or 500 mg twice daily for 1–3 days. The CDC recommends self-treatment when diarrhea is incapacitating or accompanied by fever or bloody stools. Having a course on hand allows travelers to begin treatment immediately.
The CDC notes that fluoroquinolone resistance is increasing in South and Southeast Asia, where Azithromycin is preferred. For all other destinations — including Africa, Central/South America, and the Middle East — Ciprofloxacin remains a first-line option.
